Peroneal tendinitis occurs in patients that generally have a pes cavus type of foot which puts excessive stress on the peroneal tendons. The peroneal tendons normally function to evert the foot but in a pes cavus foot the peroneals get tired from constantly trying to evert a foot that is in excessive supination. Peroneal tendinitis occurs in repetitive ankle motions in sports such as running, walking and jumping. Patients usually feel the pain behind the lateral malleolus during push off and when they rise on their tippy toes. Pain usually occurs beneath the cuboid or distolaterally to the calcaneus.

Typically individuals who do not gradually build-up tolerance to sports or walking, running and all of a sudden show an increase in activity is when they experience this problem. Treatment of this usually occurs by: PRICE (protection, ice, rest, compression and elevation), NSAIDs and physical modalities including TENs, ultrasound and laser therapy. Individuals may also benefit from a short walking boot for 2-4 weeks and a lateral wedge to take pressure off the peroneals. Individuals should also be advised to appropriately warm-up and do muscle strengthening to prevent this from occurring. Individuals may also benefit from orthotics to help support the foot and prevent excessive pressure in certain areas.
