DOCTOR HOWARD LIEBESKIND, A PODIATRIST BASED IN LOS ANGELES WAS DISCUSSING THE OPEN TRAMPOLINE INJURY TO NEW YORK YANKEES PITCHER'S RIGHT ANKLE.  THERE ARE A WIDE VARIETY OF OPEN DISLOCATIONS TO THE ANKLE AND CAN RANGE FROM CAREER ENDING TO SOMETHING THAT SIMPLY SETS AN ATHLETE BACK FOR A PERIOD OF TIME.  THIS INJURY IS GOING TO SET HIM BACK AS HE IS ALSO IN A PROGRAM TO REHABILITATE AN ELBOW INJURY AND THIS WILL CERTAINLY COMPLICATE THINGS.  HE WONT BE ABLE TO TO CONTINUE STRENGTHENING HIS ARM AT THE PROFESSIONAL LEVEL WITHOUT A FUNCTIONAL RIGHT ELBOW.  IT WON'T BE EASY.

MOST FOOT SPECIALISTS INCLUDING FOOT DOCTORS, CHIROPODISTS AND PODIATRISTS AGREE THAT IT IS IMPORTANT TO HAVE ANY INJURIES TO THE FOOT OR ANKLE LOOKED AFTER BY A PROFESSIONAL IN ORDER THAT IT IS NOT MORE SERIOUS AND CAN HEAL AS QUICKLY AS POSSIBLE PROPERLY.  AN ASIDE, DR. HARDY SPRAINED HIS FOOT AND WAS SELF TREATING IT FOR 2 WEEKS WITH PHYSIOTHERAPY IN HIS OFFICE WHEN HE FINALLY DECIDED TO X-RAY TO CHECK FOR FRACTURE AND LEARNED THAT HE DIDN'T HAVE A FRACTURE.  HE HAD BROKEN 3 FRACTURES TO HIS METATARSALS AND TIME FOR A CAST.

TORONTO  PODIATRIST, DR. GLENN COPELAND HAS BEEN TREATING DUSTIN McGOWIN FOR PLANTAR FASCIITIS AND HIS STATUS FOR THE START OF THE SEASON IS IN DOUBT.  HE WAS FORCED TO LEAVE THE GAME DURING THE SECOND INNING ON SUNDAY AFTERNOON.

PLANTAR FASCIITIS IS INFLAMMATION OF THE CONNECTIVE TISSUE THAT BEGINS AT THE HEEL OF THE FOOT AND EXTENDS TOWARDS THE SOLE.   IT CAN BE VERY PAINFUL AND OFTEN BECOMES A NAGGING PROBLEM THAT IS DIFFICULT TO TREAT.  IT IS THE SAME AILMENT THAT TORONTO OUTFIELDER SCOTT PODSEDNIK EXPERIENCED LAST SPRING.  THE DIFFERENCE IS THAT PODSEDNIK'S CONNECTIVE TISSUE WAS COMPLETELY TORN OUT AND McGOWAN'S IS JUST INFLAMED, WHICH SHOULD HELP EASE THE RECOVERY PERIOD.

MOST FOOT SPECIALISTS, INCLUDING FOOT DOCTORS, CHIROPODISTS AND PODIATRISTS AGREE THAT HEEL PAIN AND PLANTAR FASCIITIS IS ONE OF THE MOST COMMON AILMENTS WE SEE IN PRACTICE.  IN OUR OFFICE WE USE SHOCKWAVE THERAPY, INTERFERENTIAL THERAPY, LASER THERAPY, ULTRASOUND, A.R.T. (ACTIVE RELEASE THERAPY), CRYOTHERAPY, MYOPERCUSION, STRETCHING AND EXERCISE.  SOMETIMES CORTISONE HAS TO BE INJECTED WHICH WE DO BY MEANS OF COMPUTERIZED PAINLESS INJECTIONS.  ONLY IN RARE CASES IS SURGICAL INTERVENTION NECESSARY.  USUALLY THE INJURY IS A RESULT OF IMPROPER BIOMECHANICS, FOOTWEAR OR OVERUSE.  IN THOSE CASES THE PATIENT HAS TO BE PRESCRIBED CUSTOM ORTHOTICS IN ORDER TO RESTORE THE NORMAL MECHANICS AND HELP MAKE SURE THAT THE INJURY DOES NOT REOCCUR.

THE ACRONYM RICE IS A PATIENT'S AND DOCTOR'S BEST FRIEND.  RICE STANDS FOR REST, ICE, COMPRESSION AND ELEVATION.  WHEN OUR BODIES ENCOUNTER A PHYSICAL INJURY TO A JOINT, LIGAMENT, MUSCLE, ETC. THE BODY TENDS TO OVERREACT WHEN IT COMES TO INFLAMMATION.  INTENDED TO BE A PART IN THE HEALING PROCESS, UNFORTUNATELY IT NEARLY ALWAYS IS MORE THAN IS NEEDED AND TENDS TO BE COUNTER PRODUCTIVE TO THE OVERALL PROCESS.  THIS OVERPRODUCTION OF IMFLAMMATORY AGENTS ACTUALLY CAUSES TISSUE IRRITATION, DELAYED HEALING AND CAN LEAD THE FORMATION OF SCAR TISSUE AND OSTEOARTHRITIS INSTEAD OF NATURAL HEALING WITH THE ORIGINAL TISSUE.

ALL OF OUR TREATMENT IS DIRECTED TO REDUCE INFLAMMATION.  THIS IS DONE BY RESTING THE AREA OR JOINT SO AS NOT TO FURTHUR DAMAGE IT WHILE IT IS HEALING.  THE APPLICATION OF ICE  FOR 15 MINUTES A MINIMUM OF 3 TIMES PER DAY.  THIS IS ONE OF THE MOST IMPORTANT THINGS TO AID HEALING.  THE MORE OFTEN IN A DAY YOU CAN ICE THE BETTER.  THE NEXT THING THAT WE DO IS COMPRESS THE AREA WITH A TENSOR BANDAGE OR SOME OTHER TYPE OF ELASTIC SUPPORT.  THE LAST PART IS ELEVATION.  WE WANT THE INJURED PART OT BE ELEVATED TO A POSITION WHERE THE INJURY IS HIGHER THAN THE HEART.  GRAVITY CAUSES POOLING, ESPECIALLY IN THE LOWER LIMB.  IF FOR EXAMPLE IT IS YOUR ANKLE THAT IS INJURED, YOU WOULD SIT WITH YOUR FOOT ON A DESK OR IF YOU ARE LYING DOWN, YOU WOULD ELEVATE YOUR FOOT ON A PILLOW.  MANY OF THESE PROCEDURES CAN BE DONE CONCURRENTLY.  THEREFORE YOU COULD REST, ELEVATE AND ICE AT THE SAME TIME.

THE HARDEST THING TO DO IS TO GET PATIENTS TO PARTICIPATE IN THEIR OWN TREATMENT.  THEY WILL DRIVE TO YOUR OFFICE, PAY FOR THE TREATMENT, BUT ASK THEM TO DO R.I.C.E. OR EXERCISES AND MANY JUST CAN'T FIND THE TIME OR BE BOTHERED.  MOST FOOT SPECIALISTS, INCLUDING FOOT DOCTORS, CHIROPODISTS AND PODIATRISTS AGREE THAT IF THE PATIENT BECOMES AN EQUAL AND RESPONSIBLE PARTNER IN THEIR TREATMENT AND TAKE RESPONSIBILITY FOR THEIR OWN HEALTH, POSITIVE OUTCOMES  ARE GREATLY INCREASED.

MOST FOOT SPECIALISTS, INCLUDING FOOT DOCTORS, CHIROPODISTS AND PODIATRISTS AGREE THAT ANKLE INJURIES SHOULD NOT BE TAKEN LIGHTLY.  MANY TIMES PATIENTS  SELF TREAT THE INJURY WITHOUT CONSULTING THEIR FOOT DOCTOR.  MANY TIMES THE INJURY IS MORE THAN A SIMPLE ANKLE STRAIN OR ANKLE SPRAIN.

 FYI A SPRAIN IS AN INJURY TO THE LIGAMENT OR HOLDING ELEMENTS OF THE BONES AND ATTACH BONE TO BONE.  SPRAINS INVOLVE AN INJURY TO A MUSCLE OR TENDON.  TENDONS ATTACH MUSCLE TO BONE.

 95% OF ALL SPRAINS OCCUR ON THE LATERAL ASPECT (OUTSIDE) OF THE ANKLE.  THE REASON FOR THIS IS THAT THESE LIGAMENTS ARE INHERENTLY WEAK, THEREFORE MORE INJURIES.  THESE INJURIES ARE GRADED INTO THREE CATAGORIES DEPENDING ON THEIR SEVERITY.  IT IS NOT UNCOMMON TO ALSO HAVE A FRACTURE OF THE FIBULA.  SOMETIMES THESE  INJURIES REQUIRE SURGICAL INTERVENTION.

 TOMORROW I WILL DISCUSS TREATMENTS THAT ARE AVAILABLE IN OUR CLINIC AND SOME OF THE RECOMMENDATIONS WE GIVE  OUR PATIENTS REGARDING HOME CARE OF THE INJURy TO BE DONE CONCURRENTLY WITH THE DOCTOR'STREATMENT.

WELL WE DIDN'T GET WINTER AND NOW IT LOOKS LIKE WE SKIPPED SPRING.  I KNOW, THE GOOD TIMES WON'T LAST, BUT WHAT A GREAT START TO OUR COLD CANADIAN SPRING.  ALL OUR SPRING BULBS HAVE BLOOMED AT OUR RANCH.  I HOPE THAT THEY AREN'T IN FOR A CRUEL SURPRISE.

 I SEE SO MANY PEOPLE OUT ON THEIR BIKES, BLADES AND JOGGING WITH THIS GREAT WEATHER.  REMEMBER TO START YOUR ACTIVITIES AT A SLOW PACE IF YOU AREN'T USED TO THEM DUE TO WINTER INACTIVITY.  OTHERWISE YOU MAY UP INJURED AND NOT BE ABLE TO ENJOY YOUR SPRING AND SUMMER ACTIVITIES.
